- The procedure (or a photo of the itemized US treatment plan — codes visible if possible)
- Any quote amounts you’ve received
- Rough timing (“this year” is plenty)
- Anything relevant about your case — teeth missing a long time, prior work, health headlines
What not to send: detailed medical records. A procedure name and a number is all the desk needs; clinical files belong with the licensed provider who treats you.
